高血脂定義&藥物治療 8610 2013-06-25 20:17:37
一、 定義
指的是血漿膽固醇濃度偏高、三酸甘油脂值過高或兩者均偏高(王、吳，2003)，總膽固醇≧240mg/dl、三酸甘油脂≧200mg/dl(陳、梁、李、呂，1999)。
二、 症狀
高血脂通常無任何症狀。
三、 病因
近年來國人飲食有精緻化的趨勢，食物攝取以高脂肪、高膽固醇的食物為多，加上國人普遍缺乏規律運動(呂，2008)，慢慢地造成身體內代謝失調，也就形成疾病。
四、 診斷
血脂異常的診斷必須依據至少2次，且間隔1-3週的抽血檢查，和一致的血脂分析結果來判斷。
五、 治療
(一) 非藥物
1. 減輕過重體重：均衡飲食、限制熱量、運動，原則上每天消耗熱量大於攝取熱量500大卡，每週即減少3500大卡，大約可減輕體重0.5公斤，持續四週可減少2公斤(王、吳，2003)。
2. 規律有氧運動：又稱為耐力性運動，可以促進或維持心肺適能，亦可有效地改變身體與血脂組成，如快走、慢跑、游泳、騎自行車、有氧舞蹈等，運動時間每次20~60分鐘、運動頻率每週3-5次(王、吳，2003)。
3. 飲食療法：在開始藥物治療前，應先積極地嘗試3-6個月的飲食治療，對於大部分的高脂血症，一般皆能獲得相當的效果，日常生活飲食必須營養均衡且熱量足夠，達到並維持理想體重(王、吳，2003)。
(二) 藥物
1. HMG CoA Reductase Inhibitors (Statins)
是抑制肝臟合成膽固醇時的速率限制酵素，減少膽固醇的生合成，也會增加肝臟 LDL 受器的數目促使 LDL 的分解代謝。副作用主要為頭痛、失眠、肝功能異常、肌肝酸激酶 (CK) 上升及腸胃道不適。由於膽固醇合成的尖峰時間在清晨，因此 statins 建議於下午約傍晚時刻服用的效果最好(蔡、陳、張、鄭，2012)。
2. Fibric Acid Derivates (Fibrates)
為過氧化體增生激活受器 (peroxisome proliferators-activator receptor, PPARα) 致效劑，透過增加周邊組織脂蛋白脂解酶 (lipoprotein lipase) 的產生，進而促進 TG 的分解，也會增加 HDL 之合成及肝臟 LDL 受體對 LDL 之親和力，而提高 LDL 之清除。肝酵素值提高是最常見的副作用(蔡、陳、張、鄭，2012)。
3. Bile Acid Sequestrants 膽酸結合劑
作用主要是在小腸中與帶陰性電荷並含豐富膽固醇的膽酸結合，由於樹脂無法被吸收，因而增加排除。肝臟也會代償性的利用膽固醇而提高膽酸的產生，因此增加膽固醇的代謝與利用，但因會代償性增加 TG 約3-10%，所以只建議用於第二型高血脂症患者。便秘是導致患者停藥的最主要原因(蔡、陳、張、鄭，2012)。
4. Nicotinic Acid Derivatives (Niacin)
為維生素 B3，藉由抑制脂肪組織的脂解作用降低血中游離脂肪酸，並減低肝臟 TG 的合成、抑制 VLDL 變成 LDL，也可以透過抑制肝臟對 HDL 的再吸收而增加血中之 HDL 值。副作用會刺激尿酸從腎小管排出，易造成高尿酸血症、增加胰島素阻抗 (insulin resistance) (蔡、陳、張、鄭，2012)。
5. Cholesterol Absorption Inhibitors
能抑制小腸刷狀外緣運輸蛋白 (brush boarder transporter) 而減低膽固醇在腸胃道的吸收(蔡、陳、張、鄭，2012)。
6. Omega-3 Acid Ethyl Esthers
FDA 核准為高 TG 所使用的藥物，可輔助 nacin 及 fibrates 的效果。其機轉尚未清楚，有三種可能：(1) 是抑制 TG 生合成的關鍵酵素，並調控肝細胞核質受器 (nuclear receptors) 而增加脂肪酸的氧化代謝。(2) 其氧化與 TG 生合成時相互競爭利用脂肪酸物質。(3) 增加脂蛋白酶的活性因而增加 TG 的清除9,10。耐受性相當良好，副作用為有魚腥味及打嗝，需特別注意的是，omega-3 會顯著地增加 LDL(蔡、陳、張、鄭，2012)。
7. 合併藥物
其它降血脂藥與 statins 合併治療對血脂的效果，結果顯示 statin 類與 fibriates、nacin 及 ezetimibe 併用時的效果較佳，其它合併治療則沒有研究或缺乏正向的結果。複方降血脂藥物強調增加療效外，也增加服藥順從性並減少藥品費用(蔡、陳、張、鄭，2012)。

When I do not want to have a shower, neither shame me nor scold me… Remember when I had to chase you with thousand excuses I invented, in order that you want to bath… When you see my ignorance on new technologies…give me the necessary time and not look at me your mocking smile… I taught you how to do many things…to eat good, to dress well …to confront life… When at some moment I loss the memory or the thread of our conversation…let me have the necessary time to remember… and if I cannot do it, do not become nervous… as the most important thing is not my conversation but surely to be with you and to have you listening to me... If everI do not want to eat, do not force me. I know well when I need to and when not. When my tired legs do not allow me walk…give me your hand …the same way I did when you gave your first steps. And when some day I say to you that I do not want to live any more…that I want to die…do not get angry …some day you will understand… Try to understand that my age is not lived but survived.
